Twenty-five years after they climbed over the wreckage of the World Trade Center, the responders who dug through Ground Zero are in their mid-50s and older, and researchers are still measuring what the work did to them. A new brain-imaging study offers one of the more sobering readings yet: among responders living with chronic PTSD, the brain itself appears to be aging faster than the calendar.

Dr. Roberto Lucchini and his colleagues analyzed MRI scans from 99 responders, 47 with chronic post-traumatic stress disorder and 52 without. Using a deep-learning model trained on more than 11,000 scans to estimate a brain's biological age, they found that responders with PTSD had brains that looked, on average, about three years older than their actual age. The responders without PTSD showed the reverse, with brains that read slightly younger than expected. The work was published in the journal Translational Psychiatry.

The responders breathed a cloud of pulverized debris that carried carcinogens, neurotoxic compounds and other contaminants, and they did it while carrying the psychological weight of searching for the dead. ## The Longer They Worked at the Site, the Wider the Gap

The amount of time a person spent at Ground Zero mattered. Among responders with PTSD, those who worked longer at the site showed the larger increases in estimated brain age, a dose-response pattern that ties the exposure more tightly to the finding. About 23 percent of the men and women who took part in the rescue and recovery live with persistent, clinically significant PTSD, according to the study, so this is a question about a large share of the cohort, not a rare few.

## A Single Snapshot of 99 Responders' Brains

It is worth being clear about what the study can and cannot say. It is a single, cross-sectional snapshot of 99 people. It shows an association between PTSD and older-looking brains; it does not prove that PTSD causes cognitive decline, and it did not follow the responders over time to see whether the three-year gap turns into real memory loss. Lucchini frames the finding as a reason to keep watching, not a verdict. “Do not underestimate the potential health problems, physical and mental, that can be generated by any type of disaster, even decades after it occurs,” he said. ## Dementia and Frailty Are Rising Across the Same Group

The brain study lands amid a run of research pointing the same way.
